What is a Geriatric Care Manager?

Geriatric care management is the process of coordinating and planning the care of elderly individuals with intricate health needs, aiming to improve their quality of life, maintain independence, and address both medical and social aspects of their care by working with them and their families to identify needs, access services, and develop a comprehensive care plan; essentially acting as a professional advocate/professional family member to navigate the healthcare system on their behalf.

Heather L. Cash
CGCM, BS, CDP, CNA

Heather is the founder of Daughter for Hire, a Senior Advocacy/Geriatric Care Management company located in East Tennessee. She has worked in the Senior Living Space in Independent Living, Assisted Living, Memory Care and Skilled Nursing and Rehab for over 20 years before deciding to follow God’s calling to start Daughter for Hire. Her inspiration was working with seniors who would tell her that she was like “a daughter” to them.  

Another inspiration was her grandmother, Kathleen. Kathleen had been living in her home alone for many years. After a fall, she was taken to rehab and then assisted living. As the oldest grandchild, Heather used her years of experience to help her grandmother find an assisted living facility and helped get her affairs in order to make sure she could continue to have the resources needed to maintain her new living situation.

Knowing that the aging process often takes many twists and turns, it’s important that her clients can continue to have their choices heard, while working to have the best quality of life. Heather loves to build strategic partnerships in her local community and loves to learn about valuable resources that will help put the pieces of the health care puzzle together. Her focus will be on being a problem solver and advocate for each of her clients in health care, safety, social/emotional and financial decisions to work towards the best outcomes.

Heather graduated from the University of Massachusetts, Amherst with a Bachelor of Science and has been trained as a Certified-Geriatric Care Manager, Dementia Practitioner, and Nursing Assistant. Along with 20 years of experience working directly with the geriatric population, she hopes to provide the best services to her clients and families and to fulfill her calling as a Senior Advocate and a Daughter of the King.

Heather lives in Jonesborough, TN with her husband, Ross of 20 years; son, Colby (17), her 84-year-old father-in-law, Bud; and their 2 dogs. Heather and her family love the mountains of E. Tennessee, hiking and exploring their beautiful area.

Kathryn Speropulos Gray, BSN, RN
Registered Nurse | Case Manager | Patient Advocate

With over 37 years of nursing experience, Kathryn Speropulos Gray is a dedicated and compassionate healthcare professional committed to patient-centered care. Her extensive background spans acute care, home health, case management, and telehealth, with expertise in coordinating complex patient needs across multidisciplinary teams.

Kathryn’s career began at Vanderbilt University Medical Center, where she provided direct patient care in surgery and trauma. She later played a pivotal role in launching one of Tennessee’s first hospital-based home health agencies. As a case manager, she has worked with renowned institutions such as United Healthcare, Sentara Home Care, and Wellmont Health System, ensuring seamless patient transitions, reducing hospital readmissions, and improving overall health outcomes.

A skilled communicator and patient advocate, Kathryn excels in care coordination, problem-solving, and health education. Her commitment to service extends beyond clinical settings—she has participated in multiple international medical missions, providing care to underserved communities in Venezuela and Guatemala.

Kathryn holds a Bachelor of Science in Nursing from East Tennessee State University and maintains an active multi-state RN license. She continues to bring integrity, diligence, and enthusiasm to every aspect of her work.
